Update Snow's cholera map with the original.#49

The current image used in the foreword seems to be Tufte's recreation, and not Snow's original. It would be good to either mention that, or use the original one created by C.F. Cheffins using bars to represent deaths.

    The original image created by Cheffins would be this one (http://matrix.msu.edu/~johnsnow/images/online_companion/chapter_images/fig12-5.jpg), right?
