Menu: active toggles (D, Q, U, active slot) highlighted in orange, not in the focus blue #114

With D on, its button gets a blue frame — the same blue as the focus selector of the remote (#91). It looks as if D were selected, and with a remote it is hard to tell which button has the focus.

• The "on / active" highlight (D on, Q in multi-view, U in the U view, the active slot in single view) gets a different colour, e.g. orange, and stays visible while the focus moves.
• The focus frame stays blue.
• A button that is both active and focused shows both (e.g. orange fill or thick orange border + blue focus frame), so neither state hides the other.
